Bowel cancer before 50 may not be the same disease: Here’s what happens when it spreads to the liver

A major Australian study of 1,691 patients with advanced bowel cancer has found that younger adults whose cancer had spread to the liver showed important differences in tumor characteristics, genetic mutations and survival compared with patients diagnosed after age 50. A large Australian study found important differences between younger and older adults with advanced bowel cancer. Researchers analyzed 1,691 patients whose cancer had spread only to the liver. Patients diagnosed at 50 or younger were more likely to be women, have left-sided tumors and carry certain genetic mutations. They also had better overall survival than older patients after the cancer spread to the liver.

The study was led by Flinders University and Flinders Medical Centre and published in the Medical Journal of Australia.

Why the liver matters in advanced bowel cancer

The liver is the most common place for bowel cancer to spread. Researchers found that patients who had liver surgery followed by drug treatment generally had better outcomes. However, the study was based on routine clinical data, not a clinical trial, so it cannot prove that surgery itself caused longer survival.

BRAF and KRAS mutations linked to poorer outcomes

The researchers found that BRAF and KRAS mutations were associated with poorer outcomes in both younger and older patients.

The findings highlight the potential importance of genetic testing when doctors assess advanced bowel cancer.

Professor Savio (George) Barreto of Flinders Medical Centre said: "We still have much to learn about why these cancers develop and how they differ from those diagnosed in older people. Our study helps build a clearer picture of the disease and suggests that younger patients may have distinct tumor characteristics that should be considered when treatment decisions are being made."

Age is not the only factor

The researchers say treatment decisions should consider more than a patient's age, including tumor genetics, cancer spread and overall health. "Our findings reinforce the need to look beyond a patient's age when planning treatment because every cancer is unique," says Professor Barreto.

"The more we understand about the biology of a tumor, the better equipped we are to tailor treatment and identify the options most likely to benefit each patient."

Why younger bowel cancer is a growing concern

The findings come as researchers investigate the rise of early-onset bowel cancer, with more adults being diagnosed before 50. Understanding why younger people develop bowel cancer and how their tumors differ could help researchers develop more personalized treatment strategies.

The study adds to evidence that younger-onset bowel cancer may have distinct biological features. Researchers say future treatment decisions could increasingly combine tumor genetics, disease characteristics, overall health and treatment response.

"Treatment decisions should take into account a person's overall health, the extent of their cancer and the unique characteristics of their tumor," says Professor Barreto.

"Better understanding of these differences is an important step towards delivering more personalized care and improving outcomes for patients and their families."
